UA First Semester 2025-2026 Freshman Enrollment (UASHS graduates)

Greetings from the UA Office of the University Registrar!

The University of the Assumption gladly welcomes you to the First Semester Academic Year 2025-2026.

To avail yourself of the University’s services, you need to be officially enrolled. Kindly take note of the enrollment schedule, procedure, policies, and guidelines for your guidance.

COLLEGE FRESHMAN EARLY ENROLLMENT SCHEDULE

Schedule:
Starting May 19- July 4, 2025 (8:00 am – 12:00 pm, 1:00 pm – 4:00 pm)
EXCEPT on May 23, May 30, June 6, June 12, June 16 (morning), June 20, June 18 for Nursing and Pharmacy, and June 24 -27, 2025)

Venues:
OUR, Dean’s Office (SAS, SEd, SBPA, COA, CONP, CHTM, CEA, CITCLS), Finance Office, Photo Lab, OMIS, and RSO Office

DRESS CODE:
Comfortable but decent clothes (pair of jeans/slacks, white shirt, rubber sneakers/closed shoes).
Please enroll during the prescribed schedule to be assured of your slots in classes.

ENROLLMENT PROCEDURE

PRESENT the Letter of Acceptance with Student Number sent via email by the Guidance and Admissions Office (GAO) (to be presented to the enrollment personnel at the Step 1 – Office of the University Registrar). If you have not been issued the Letter of Acceptance, you may proceed to the GAO. (First floor, Ryan Building).

PRINT the following documents for enrollment purposes:

  1. 1st-sem-2025-2026-Enrollment-Schedule-and-Procedure
  2. 1st-sem-2025-2026-UA-College-Freshman-Enrollment-Routing-Form (from May 19 – July 4, 2205 – as procedural guide to be signed by enrollment personnel per Office)
  3. Form UA-OUR-30 Student Information Sheet (to be submitted at Step 1)
  4. Affidavit of Enrollment Undertaking (to be submitted at Step 1 if enrollment requirements for submission are incomplete)
  5. College Enrolment Site (map) per Office (from May 19- July 4, 2025)
  6. UA-College-Uniform-Pattern

COLLEGE ENROLLMENT STEPS

The detailed steps of the College Enrollment are specified in the 1st-sem-2025-2026-UA-College-Freshman-Enrolment-Routing-Form

STEP 1 - SUBMISSION OF ENROLLMENT REQUIREMENTS - Office of the University Registrar

The following requirements have to be submitted:

  1. Original (hard) copy of SHS Form 138 Grade 12 card
  2. Original (hard) copy of Certificate of Good Moral Character
  3. Clear Photocopy of the PSA Birth Certificate/Marriage Certificate
  4. Recent Identical 2” x 2” Photos (2 copies) with white background and name tag (Last Name, First Name, Middle Initial)
  5. Accomplished UA Student Information Sheet – The form is attached to this email.
    Download, accomplish via Adobe Acrobat Reader, and print out the form. You may also handwrite your entries in the form.
    *If requirements are INCOMPLETE, download, accomplish via Adobe Acrobat Reader , and print out the AFFIDAVIT for the APPROVAL of the REGISTRAR.
  6. *Accomplished Affidavit of Enrollment Undertaking – The form is attached in this email.
    Download, accomplish via Adobe Acrobat Reader, and print out the form. You may also handwrite your entries in the form.
  7. College-Enrollment-Evaluation-Form
    You will be issued the College-Enrollment-Evaluation-Form . Kindly evaluate the enrollment process, venue and personnel. Your inputs are valuable for the improvement of college enrollment. Submit the accomplished evaluation form at Step 6.

STEP 2 - ENLISTMENT IN COURSES - College Offices

You will be enlisted in courses/subjects and provided with the CLASS SCHEDULE/ ASSESSMENT FORM with the schedule of courses, venues, and professors, including assessed fees for the semester.

STEP 3 - PAYMENT OF FEES- Finance Office

Upon paying the fees for the semester (initial or full fees), you will be issued the OFFICIAL RECEIPT. The initial payment for College is Php 5000.00, except for Nursing and Pharmacy Php 8000.00

If you have already paid Php 1000.00 reservation fee, you will just be paying Php 4000.00 (other programs) or Php 7000.00 as initial payment (BSN, BSP). The schedule of payment is:

Initial payment – upon enrolment
Prelim payment – before the prelim exam
Midterm payment -before the midterm exam
Final payment – before the final exam

Kindly stand by for the posting of the tuition, miscellaneous, and other school fees by the Finance Office.

STEP 4 - ID CARD PROCESSING - Photo Lab

Your image will be taken for the issuance of your UA ID card.

STEP 5 - UA EMAIL ISSUANCE - OMISS

Your UA email will be ACTIVATED.

STEP 6 - COLLEGE ORIENTATION - RSO Office

Attend the orientation to be informed of college policies, guidelines, and procedures. Submit the Accomplished College-Enrollment-Evaluation-Form. The College Student Attire will be discussed in detail by the Director of Student Affairs, Mrs. Precious Bernadette Estrada.

ENROLLMENT POLICIES and GUIDELINES

You will be considered OFFICIALLY ENROLLED if:

  1. All enrollment requirements have been submitted to UA
  2. Initial or full payment of school fees for the semester has been paid
  3. The name of a student is included in the Official Class List generated from the UA School Enterprise Resource Planning (SERP)

The enrollment process should be completely accomplished before attending the first day of classes on August 1, 2025. You need to be officially enrolled to attend and participate in classes, e.g., join groups, submit papers, take quizzes or exams. Only students whose names appear in the official class list generated from the UA School Enterprise Resource Planning (SERP) shall be admitted to class. Only students who are officially enrolled are entitled to grades at the end of the semester or term. All credentials submitted for enrollment purposes become part of the University records and can only be withdrawn within the first two weeks of classes. If any special circumstances prevent you from completing the registration process, you may personally visit me at the Office of the University Registrar.

Keep the following for enrollment verification purposes:

  1. Class Schedule/Assessment Form stamped ENROLLED
  2. Official Receipt stamped ID issued
  3. Enrollment Routing Form

The regular Freshman enrollment is from July 7-11, 2025, at the Abp Cinense Gymnasium. If you will enroll that week, please print the following documents:
